The lyrics were inspired by observation of their young children - a motif evident in the lyrics "Dangling feet from window frame / Will they ever ever reach the floor / More give me more give me more". The single, like the rest of the tracks on Fever Ray, was written by Dreijer themself. Dreijer teamed up with long-term production partner Christoffer Berg (well known for his work with The Knife) for the production of "If I Had a Heart".

Another comment was the lyrics are about addiction. The song takes on a generally unconventional, dark, and experimental style with its slow tempo, organ backing, repetitive synth beat, and distinct vocals. There was also an explanation that Karin Dreijer Andersson (Fever Ray - the group who did the song) wrote it about her own children. It opens your mind slightly to the endeavors of what death would potentially be thinking, should death personified exist. This song, being written and sung for the show should guide you with the standing of the show itself. Nothing will ever fulfill it, so it searches for more hence the "More, give me more, give me more". In the context of the series Vikings, it portrays the eternal hunger (in my opinion) of death. The child singing is also said to symbolize death's inner child and wanting to escape the role and life he was chosen for. It is all meant to fit in as a whole and not taken out of verse context. If he had a voice he would sing, symbolizing that he would show his emotions if he had a heart, if he had a voice. He has no heart, so he cannot love, hence people die. It was said by others to show that death is never satisfied with all it gets, he keeps wanting more. The hypnotic tune is supposed to lure your consciousness in and the low key singing is supposed to signify death himself. "I've also read that this some is supposed to have a symbolic meaning of death. The following are not my words but from Metrolyrics: Don't know if it's true but it is plausible. I read an explanation, actually many explanations, on Metrolyrics.
